Step 1:
Open sketch on Front plane.

Draw two lines as shown on the pricture below.

Add dimensions.

Add sketch fillet.

Step 5:
Close the sketch and open new one on the Top plane.

Step 6:
Draw a polygon with six sides.

Add dimension.

Close the sketch.

Click on Swept boss/base in Features.

Click sketches as shown on the picture below and accept feature.
