Eduardo you should upload all the part files and the assembly .iam file in a compressed format (zip, rar, 7z), not just the assembly file.

Hi,
Just Answering, hoping it will be useful for some one who search for the same
Step:
1) use Motion Constraint - "Rotation-Translation Constraint" for screw to rotate based on jack up and down movement....
2)Then use "Drive constraint" option to drive top Plate...its very simple....
